3. Arrival at the Hospital

Please contact the Registration Dept. on the second floor of the hospital

1. if you are a new patient, please fill-in your personal information and

• Inform us of any allergies you may have such as food or drugs
• Inform us of any insurance or company benefits you have
• Inform us if you need a medical certificate or a medical report

2. if you are a current patient, please give the staff your appointment slip or inform the staff of your appointment number

• Inform us of any changes to your name, address or contact numbers
• Inform us if you need a medical certificate or a medical report
