1. Participation in the package is subject to the ophthalmologist's discretion.

2. This package includes operating room and recovery room charges, medical supplies, surgical medications, fees for the medical team performing the procedure, and anesthesiologist service charges related to the surgery and post-operative care only.

3. If the ophthalmologist recommends the use of lenses other than those specified in the package, additional charges will apply.

4. This package excludes doctor fees, consultation fees, pre-operative evaluation fees, and other expenses not specified in the package. Additional charges may apply in cases requiring further investigations, treatment of unrelated abnormalities, or management of other eye conditions not directly associated with this procedure.
